ASL 1 class registration form

This registration form is for ASL 1, which will meet from September 22nd to November 10th with the 17th being a make up day.

Please fill out registration form before September 10th. Please mail/or pay in person before September 10th. You may also make arrangements to pay in cash before September 10th. Class fees on a sliding scale: $175-250. Cash/check only. No refunds will be given after September 20th.

If a person is younger than 14, I may make some exceptions. However, the child, parent and I will need to have a discussion beforehand. This class is more for teens and adults. I am willing to set up a different type of class for those who are younger.
