ASL 1 class registration form

This registration form is for ASL 1, this class will meet February 5th- April 2nd, With April 9th as a make up day. There will be no classes during February school break (Feb 19th). This class will meet from 6:00-7:30.

Please fill out registration form with payment before January 25th. You may also make arrangements to pay in cash before January 25th. Class fees on a sliding scale: $175-250. Cash/check only. No refunds will be given after Friday, January 30th.

If a person is younger than 14, I may make some exceptions. However, the child, parent and I will need to have a discussion beforehand. This class is more for teens and adults. I am willing to set up a different type of class for those who are younger.

I have mailed/given payment of $175-250 (sliding scale) and am able to commit to the 8 weeks of class. If I am not feeling well, I will not show up, but I understand that there will be no refund for missed class.(required)
I understand that the instructor is not responsible for any illness or injury. (required)
I understand that the instructor may need to cancel class (illness/weather), and I will go with the majority of the class votes in either a refund/make-up date/online class to make up for the cancelled session. If the instructor needs to cancel the remaining classes, the amount will be pro-rated. (required)
