ASL 1 class registration form

This registration form is for ASL 1, this class will meet September 21st through November 16th. There will be no class October 12th. The class will take place from 4:00-5:30.

Please fill out registration form before September 14th, its important that I have your name by this time for your materials to be printed.

Please fill out registration form with payment before September 14th. You may also make arrangements to pay in cash before September 14th. Class fees on a sliding scale: $175-250. Cash/check only. No refunds will be given after Friday, September 18th. This class does fill up quickly, keep in mind that your spot is not reserved until I recieve payment. Please also email me to let me know how you will be paying and to watch for it in the mail.

If a person is younger than 14, I may make some exceptions. However, the child, parent and I will need to have a discussion beforehand. This class is more for teens and adults. I am willing to set up a different type of class for those who are younger.

I have mailed/given payment of $175-250 (sliding scale) and am able to commit to the 8 weeks of class. If I am not feeling well, I will not show up, but I understand that there will be no refund for missed class.(required)
I understand that the instructor is not responsible for any illness or injury. (required)
I understand that the instructor may need to cancel class (illness/weather), and I will go with the majority of the class votes in either a refund/make-up date/online class to make up for the cancelled session. If the instructor needs to cancel the remaining classes, the amount will be pro-rated. (required)
I agree to try my best to not to talk in class. I am at least 14 years old.(required)
