ASL 2/2.5 class registration form

This registration form is for ASL 2, Which will meet February 5th- April 2nd with April 9th as a make up day. There will be no classes during February school break (Feb 19th). This class will meet from 4:00-5:30.

Please fill out registration form before January 25th with full payment. Class fees on a sliding scale: $175-250. Cash/check only. No refunds will be given after January 30th.

Mail payment to: Carrie Pierce P.O. Box 12, Belfast, ME 04915. You may also pay Carrie cash in person before January 25th.
